Eligibility

Before coming to any clinic, an eligibility interview appointment is necessary to determine if a patient qualifies for services.

Eligibility interviews are conducted week days; please call 703/779-5416 to make an appointment Refer to the information below for guidelines and items to bring with you for the interview.

Upon eligibility patient will be issued a LCFC card good for six months. Patient must re-qualify every six months for eligibility.

  • Loudoun County resident (Proof)
  • Between ages 18 - 64
  • Have no health insurance
  • Income Level: 200% of Federal Poverty Level (Proof). Click HERE to view the financial requirements chart.

This Foundation was formed in 1998 as a not-for-profit, 501(c)3 organization pledged to increase access to health services for the medically under-served in Loudoun County.
